Introduction: Molecular imaging revolutionizes medical diagnostics by delving into disease processes at the molecular and cellular levels. This technique, combining advanced imaging technologies with molecular probes, visualizes and quantifies specific molecular targets in real-time. Molecular imaging holds vast potential for enhancing the accuracy and efficacy of surgical interventions, particularly in robotic precision surgery. This study comprehensively reviews the applications and challenges of molecular imaging in this context. Method: PubMed, Science Direct, Web of Science, and Google Scholar databases were explored until October 2023, using different combinations of keywords "Molecular Imaging," "Surgical Interventions," "Robotic Surgery", and "Robotic Precision Surgery". Ultimately, the 11 most recent and relevant papers were included in the study. Results: Molecular imaging offers applications from early diagnostics to therapy planning and execution in precision surgery. Molecular imaging facilitates detailed preoperative planning. It provides comprehensive information about tumor molecular characteristics, enhancing diagnostic accuracy for a more informed and targeted robotic surgical approach. The imaging modality allows visualization of specific molecular markers in tissues, enabling precise targeting during robotic surgery and enhancing tumor resection accuracy while minimizing damage to healthy tissues. Molecular imaging's real-time nature provides dynamic information during surgery, assisting surgeons in adapting their approach based on the evolving molecular landscape, especially in procedures where tissue boundaries are not easily discernible. However, certain modalities may have spatial resolution limitations, hindering precise molecular target localization during robotic surgery, crucial for accurate interventions. The use of molecular probes raises safety concerns, emphasizing the need for probe safety in the clinical translation of molecular imaging in robotic precision surgery. Connecting advances in medical devices with tracer-based molecular imaging strategies offers a more comprehensive view of precision surgery's direction. Conclusion: Integrating molecular imaging with robotic precision surgery marks a paradigm shift in surgical interventions, providing real-time, high-resolution information about molecular targets. This fusion holds great promise in reshaping the surgical landscape, leading to improved outcomes and reduced invasiveness. However, addressing pitfalls is crucial for successful clinical implementation. Ongoing research and technological advancements aim to overcome challenges, paving the way for a new era in precision medicine and surgery.
