The Prognostic Comparison Among Unilateral, Synchronous and Metachronous Bilateral Breast Cancer: Report from Shiraz Breast cancer Registry

Abstract Introduction: The incidence of bilateral breast cancer (BBC) is increasing nowadays. To better understand the survival of synchronous bilateral breast cancer(SBBC), metachronous bilateral breast cancer(MBBC) and unilateral breast cancer(UBC) we performed this study within a large breast cancer registry in Iran. Method: Clinicopathological features, histiopathological features, immunohistochemical findings, and adjuvant systematic therapy status of the patients with breast cancer were obtained from the Shiraz Breast Cancer Registry(SBCR) .These findings were compared between UBC, SBBC, and MBBC groups. Moreover, the overall survival(OS)and disease-free survival (DFS)of UBC and BBC groups were compared. Results:10823 patients were enrolled in the study. Tumor necrosis(P=0.008) beside the larger tumor size(P=0.001) and existence of in situ component(P=0.007)were more prevalent in UBC in comparison with SBBC and MBBC. Tumor in SBBC group were more likely to be HR positive(P=0.001) ,whilst there was no difference in terms of HER2 amongst three groups. Adjuvant chemotherapy (P=0.001)and radiotherapy(p=0/02) were more prevalent in MBBC group .The 5-year OS of patients in SBBC, MBBC and UBC , were 50%, 65% and 74 % respectively( P=0.001). 5- year DFS for SBBC, MBBC and UBC patients were 47% , 56% and 71% respectively (P= 0.001) Conclusion: SBBC patients are more likely to be HR positive and has lower OS and DFS compared to MBBC and UBC patients. Therefore, early detection and accurate diagnosis of breast cancer are crucial for determining the appropriate treatment and improving the prognosis of breast cancer.
