Introduction: Due to the rarity of Granulomatous Mastitis, and the lack of enough data, this study was designed to evaluate the patients diagnosed with GM and treated in this center with focus on the assessment of clinical, radiologic, and histologic findings. Method: This was a cross-sectional study conducted in a tertiary health care center. Records of 657 patients with histopathology records of GM who referred to Shahid Motahari breast clinic, the referral center of breast diseases in the south of Iran from 2008 till 2021 were investigated in this study. Result: The mean age of the patients was 35.38 ± 8.25, with the higher presentation at the childbearing and reproductive age. The most prevalent clinical finding was palpable mass in the upper outer quadrant of the left breast, followed by erythema and tenderness. The most frequent finding of ultrasonography was a single heterogeneous hypo-echoic diffuse area with ill-defined borders, in the central part. In mammographic assessments, the most common finding was a single diffuse area of involvement in the central part. The most prevalent type of involvement was focal asymmetry, followed by skin thickening. Amongst all patients, 54.4% were diagnosed with GM. 6.7% of our patients were diagnosed with cystic neutrophilic GM. Abscess formation was detected in 18.4% Conclusion: Clinical presentations and radiologic features of GM tumors are nonspecific and tissue biopsy is essential for a definite diagnosis. Optimal treatment seems to differ, depending on the patient’s condition and the surgeon’s decision
